Three Easy Steps to Get Started
Schedule
You must complete the form below to create an account and sign up for your initial visit with our Clinical Care Coordinator. In preparation for the meeting, the clinical care coordinator will send you the Welcome Packet to fill out before your first appointment.
Connect
You will meet with the Clinical Care Coordinator, review your Welcome Packet, and review your options for care. The Clinical Care Coordinator will connect you with your preferred provider or set you up with instructions for accessing a support group.
Meet
It’s time to get started! You will receive direct communication from a Well professional via the Patient Portal in order to connect to your first appointment or group program.
In order to get started at Well, we need a few details from you! Please fill out the following form to schedule your first appointment. You will receive instructions via email to create an account in our patient portal prior to your appointment along with an Intake Packet.
